To complete an application, individuals must bring the following:

Last four weeks of pay stubs or proof of income
Social Security numbers of all household members
Proof of citizenship or Legal Permanent Resident Card
Documents showing other sources of income such as social security, Supplemental Security Income, veteran’s benefits, or child support
